You settle the costs with your treating physiotherapist as an optional therapist and apply to your competent health insurance provider for partial reimbursement in accordance with the cash tariff\/statutory cost allowance. Your treatment<\/strong><\/p>\n<p>The services provided by your physiotherapist include all measures performed directly with and for you, in particular<\/p>\n<ul>\n<li>personal individual treatment including diagnosis and consultation<\/li>\n<li>treatment-related administration, scheduling of appointments<\/li>\n<li>Preparation and follow-up necessary for the treatment, e.g. production, adaptation and provision of individual therapy materials<\/li>\n<li>Documentation (medical history) and 10-year retention, whereby you have a right to inspect and copy (against reimbursement)<\/li>\n<li>if required\/on request: compilation of individual findings going beyond the documentation for submission to various bodies such as health insurance carriers, treating physicians, private insurance carriers and similar bodies<\/li>\n<\/ul>\n<p>With your signature following a treatment session, you confirm the utilization of the treatment. (MTD Act)<\/p>\n<p><\/span><\/li>\n<li class=\"pnText\"><span lang=\"DE\">Science:<br \/>\nYour physiotherapist is guided by the latest scientific findings.<\/p>\n<p><\/span><\/li>\n<li class=\"pnText\"><span lang=\"DE\">Self-determination:<br \/>\nYour physiotherapist will make a treatment proposal based on the doctor&#8217;s prescription and the initial assessment. It is up to you to accept this offer or to discuss adjustments with your physical therapist.<\/p>\n<p><\/span><\/li>\n<li class=\"pnText\"><span lang=\"DE\">Secrecy:<br \/>\nAll information you give to your physiotherapist is subject to confidentiality. It is assumed that an exchange of information for the purpose of optimizing treatment is desired with the prescribing physician as well as with the other health care professionals named by you and involved in the treatment. This information will not be disclosed to any other person without your consent. If further disclosure of information proves useful and necessary for medical-therapeutic reasons, your physiotherapist will consult with you about it. Payment mode<\/span><\/b><\/p>\n<p class=\"pnText\"><span lang=\"DE\">At the end of the treatment (or treatment sessions of the doctor&#8217;s prescription), your physiotherapist will issue you a fee note for the total cost of the treatment sessions.<\/span><\/p>\n<p class=\"pnText\"><span lang=\"DE\">You can arrange the following payment method with your physiotherapist:<\/span><\/p>\n<p class=\"pnText\"><span lang=\"DE\">&#8211; Cash payment <\/span><\/p>\n<p class=\"pnText\"><span lang=\"DE\">&#8211; Payment with payment slip\/bank transfer<\/span><\/p>\n<p class=\"pnText\"><span lang=\"DE\">You agree with your physiotherapist on the date of payment (due date) &#8211; it can be found on the fee invoice. If you are in default with the agreed payment method, your physical therapist reserves the right to charge interest on arrears at the legally permissible rate of 4%. For reminders issued in connection with fee claims not paid according to the due date, the reminder fees charged shall amount to 5 euros for the first reminder and 10 euros for the second reminder. <\/span><span lang=\"DE\">Euro<\/span><span lang=\"DE\">and for the third reminder to 20 <\/span><span lang=\"DE\">Euro<\/span><span lang=\"DE\">,-. Both you and your physiotherapist are also free to discontinue treatment at any time without giving any reason.
